Les Bell and Associates is conducting a survey on the experiences of organisations in both the private and public sectors with implementation of Free and Open Source Software. This questionnaire is loosely based on one used by the UK's Office of Government Commerce so that results should be comparable. Results will be presented in a seminar to be held in the middle of this year (2004).

Please pick one of the two questionnaires: the short-form survey should only take approximately five minutes to complete. If you are feeling particularly helpful, and have approximately 20 minutes to spare, we'd really appreciate it if you could tackle the full survey. Yes - we know it's not short, but it is reasonably comprehensive.

As an incentive, those who complete either questionnaire and provide an email address will be provided with a short report which details key findings.

Although the questionnaire asks for contact details so that we can follow up any problems, all data will be de-identified before use.
